Yasmin Pervez, MD is an Internal Medicine physician, she has been providing health care in the North Tonawanda area for over 25 years. She is conveniently located at 525 Wheatfield Street, Suite 10, the office is brand new and is spacious and handicap accessible with parking right outside the front door.
Our Office
Friendly, attentive and compassionate staff is here to help with any of your concerns. We do run on a very timely schedule and ask patients to please arrive on time for your appointments with your insurance card, medication list and a list of all the specialist that you see. Kindly contact the office within 24 hours if you are unable to keep your appointment.
Anyone interested in becoming a new patient to our practice we ask that you come in during regular business hours and fill out the New Patient packet along with a valid insurance card. Once completed we will contact you within 2-3 business days to schedule an initial appointment.
Policies
Medication Refills are done daily, please contact the office or your pharmacy if you need refills.
Any patient requiring refills on their controlled substances ( Xanax, Ambien etc) we advise you to contact the office one week prior to needing a refill. Please do not wait until the last minute as you may not get your refill until the following week.
Laboratory and Other Test Results: Any patient with abnormal tests results will be contacted to schedule an appointment to discuss further treatment if needed.
We DO NOT provide antibiotics over the phone, if you are not feeling well you MUST make an appointment, our office does provide allotted times for same day sick or urgent appointments.
This office does NOT provide pain medications on a regular basis, if you are a new patient seeking pain medications, you will be provided information for Pain Management after your appointment.
